As a Nurse Navigator at Mercy, you will play a pivotal role in guiding patients and their families through the healthcare journey, ensuring they receive timely, coordinated, and compassionate care across Mercy’s Healthcare & Medical Services. Serving as a key clinical resource and advocate, you will help patients understand diagnoses and treatment options, navigate complex care systems, and access the support and services they need. Working within Mercy’s collaborative and patient-centered culture, you will partner closely with physicians, advanced practice providers, bedside nurses, social workers, therapists, and other members of the care team to streamline the care continuum—from initial referral and diagnostic workup through treatment, follow-up, and survivorship or long-term management. You will conduct comprehensive nursing assessments, identify clinical and psychosocial needs, develop individualized care plans, and provide evidence-based education tailored to each patient’s condition, health literacy, and cultural background. Your responsibilities will include coordinating appointments and referrals, proactively tracking test results and treatment milestones, and facilitating communication among patients, families, and multidisciplinary teams to minimize delays and reduce avoidable hospitalizations. You will support patients in understanding medications, symptom management, and self-care strategies, reinforcing provider instructions and helping them recognize when to seek urgent care. You will also help patients connect with community resources, financial counseling, support groups, rehabilitation services, and palliative or supportive care when appropriate. In this role, you will utilize electronic health records to document encounters, monitor progress against care plans, and support quality and safety initiatives. You will contribute to population health and outcomes-improvement efforts by identifying care gaps, participating in case conferences and huddles, and sharing insights to refine clinical pathways and best practices.
