Mercy is seeking a compassionate, patient-focused Nurse Navigator-Specialty to join our Healthcare & Medical Services team. As a key clinical resource within our specialty care programs, you will guide patients and their families through complex healthcare journeys, helping them understand diagnoses, treatment options, and care plans while ensuring timely access to the services they need. In this full-time role, you will work collaboratively with physicians, advanced practice providers, therapists, social workers, and support staff to coordinate care across the continuum—from initial referral and diagnosis through treatment, survivorship, or ongoing chronic disease management. Drawing on your clinical expertise, you will perform focused assessments, identify barriers to care, and proactively address medical, psychosocial, and educational needs. You will serve as a consistent point of contact for patients and families, facilitating communication between the patient and the healthcare team and promoting a seamless, patient-centered experience. The Nurse Navigator-Specialty at Mercy is responsible for organizing and prioritizing patient caseloads, tracking diagnostic testing and follow-up appointments, and ensuring that evidence-based care pathways are followed. You will educate patients and families in clear, understandable language, reinforcing provider recommendations, reviewing medication and treatment plans, and connecting them with community resources and support services. Using electronic health records and navigation tools, you will document encounters, monitor progress toward care milestones, and escalate concerns to the appropriate team members when clinical changes occur.
